A verification report should let someone repeat the check. An isolated verdict, however confident, is insufficient. Fact-checker organizes claims, evidence and proposed corrections; the result depends on the sources actually consulted.
Define the review boundary
Supply the full text and reference date. If it summarizes notes or a transcript, attach them: checking documentary fidelity and checking external truth are different operations. Without web access, request a limited document comparison rather than current verification.
The pack distinguishes numbers, quotations, causal claims and comparisons. Prioritize claims that affect decisions. If time is limited, report which assertions were left unchecked.
Locate the exact evidence
For each claim, request the relevant passage, source and available dates. Check the population, period and indicator definitions. Several pages copying one press release are not independent confirmations.
An inaccessible page leaves the review incomplete. Missing evidence does not automatically establish falsehood. Separate contradiction, outdated information, missing qualification and inability to verify.
A fictional document example
The source says: “12 participants responded; 8 found the session useful.” The draft says: “All participants found the session useful.” The correction preserves the respondent group: “Among the 12 respondents, 8 found the session useful.”
This does not reveal non-respondents' views or the total group size. It shows how a rewrite can exaggerate evidence without changing its subject.
Resolve outstanding points
